Iowa One Call's FAX-A-LOCATE allows participating utilities and contractors to submit routine locate requests to IOC via fax.

• To participate in FAX-A-LOCATE, utilities and contractors must first complete the FAX-A-LOCATE Agreement and fax it to Iowa One Call at 800-238-6015.
   
• Once you have received a return fax from Iowa One Call confirming your enrollment in FAX-A-LOCATE, you may begin using the Iowa One Call FAX-A-LOCATE Request form.

After installing Adobe Acrobat Reader, or if you already have Acrobat Reader installed, simply click on one of the form links above. Print the form using the printer icon in the Adobe Acrobat Reader window, then print or type the information requested on the form. Sign the form indicating you have read and understand the contents, then fax the completed form to Iowa One Call at 800-238-6015.

Please note that you do not have to submit the Agreement more than once; however, you cannot use the Request form until you have received confirmation from Iowa One Call that you have been entered into the computer as a FAX-A-LOCATE member.
